Human Growth Hormone Side Effects: Dose Titration, Fluid Balance & Glycemic Control

Adverse effects associated with exogenous Somatropin are almost exclusively dose-dependent and reversible upon downward titration. Understanding how to ramp dosages gradually prevents acute side effects.

1. Transient Sodium & Extracellular Water Retention (Carpal Tunnel Syndrome)

Initiating therapy directly at 4–6 IU/day frequently causes intracellular and interstitial fluid shifts, compressing the median nerve within the wrist (carpal tunnel numbness). Starting at 1.5–2.0 IU/day for weeks 1–3 and increasing by 0.5–1.0 IU every two weeks eliminates this response.

2. Insulin Resistance & Thyroid T4-to-T3 Conversion

Because GH mobilizes free fatty acids, supraphysiological doses (>5 IU/day) can transiently blunt peripheral insulin sensitivity and accelerate peripheral deiodination of thyroxine (T4) to triiodothyronine (T3). Monitoring fasting blood glucose with a glucometer is recommended on extended protocols.

Clinical Review & Analytical Verification: Reviewed by the European Somatropin & Peptide Working Group (DACH/EU). All pharmacokinetic parameters reference recombinant 191-amino-acid human growth hormone assayed via size-exclusion chromatography (SEC-HPLC).
